Improving Chronic Care by Improving Care Coordination
TEAM SAN DIEGO provided an online education program that was designed to train physicians, their office staff, and community health and social service providers to work together as a "virtual team"; assist providers in helping their patients effectively participate in managing their own health and health care outcomes; and provide easy access to local community resources for patients and their caregivers.
TEAM SAN DIEGO was supported by a grant from the California Endowment and Alliance for Healthcare Foundation. It was a joint initiative of George Mason University, UC San Diego Extension, and the County of San Diego's Long Term Care Integration Project.
TEAM SAN DIEGO was the recipient of national, state and private industry awards. On July 30, 2010, TEAM SAN DIEGO received the 2010 National Association of Area Agencies on Aging (N4A) Innovation Award for Health Aging at their national conference in St. Louis. On October 19, 2010, TEAM SAN DIEGO was awarded the 2010 California State Association of Counties (CSAC) Challenge Award and on March 28, 2011, Trilogy Integrated Resources, Inc. presented TEAM SAN DIEGO with 2011 Trilogy Network of Care Innovators Circle Award for the innovative use of technology.
