May only receive assistance once every 12 months (payment must be cleared before client is eligible to re-apply), regardless of amount. 4. Household members without income must be present for walk-in appointment to sing an affidavit. 3. Proof of any assistance i.e. food stamps, cash aid,wic, etc. (if applicable) 4. ID and Social Security card (Hard Copy) (applicant only) 5. Complete copy of your current SDG&E bill (all pages) 6. A notarized power of attorney (if a family member or household member) is attending on behalf of the account owner. Anyone can mail in a request to get an application by mail.
