• Eligibility Requirements:Must be age 18 years and older, homeless or at risk of homelessness, and receiving Medi-cal or Medi-cal eligible (cannot be eligible for or receive Medicare or Medi-Medi). Must have a chronic health condition, mental health, and/or substance use disorder. Must also have 5 or more ED visits or 3 inpatient stays in the last 12 months or 2 or more ED visits in the past month. Cannot have a terminal illness with less than one year to live.. No Documents Required. Call, if no one answers leave a voice message and someone will return the call as soon as they can. Assessment is completed in the community, wherever the client can meet. Referrals are accepted from hospitals or social service providers.
