• Eligibility Requirements:Only authorized individuals may receive certified copies of birth or death certificates. Authorized individuals include parents or legal guardians, children, grandparents, grandchildren, siblings, spouses or domestic partners of registrants. In order to receive certified copies, individuals must complete and sign a sworn statement included with birth or death certificate application form (for mailed requests, the sworn statement must be notarized). Individuals not authorized receive copies marked "informational; not a valid document to establish identity.". Call for details/more information. Need a valid government-issued identification. To obtain birth certificates by walk-in, applicant must have the child's full name as stated on the birth certificate, the mother's full maiden name, the child's date of birth, and the child's place of birth. To obtain death certificates by walk-in, applicant must have decedents name and date of death.
