Coming Home/Thriving in the Community (Transition), is to empower individuals with the tools and resources to live safely in the community. Provides a Safe Community Living Assessment, create a Community Living Plan, implement Community Living Plan, move-in to new home and to ensure success. Youth in Action (Youth Transition) provides assessment and Planning Exercise, Independent Living Skills, recreation and social gatherings, Peer Support/Self-advocacy group, education planning, community involvement and employment readiness. Community Connections/Engage (Community Education) is to provide training and education which raises awareness, empowerment, and advocacy. Conduct survey to gather topics of interest, provide ongoing groups and workshops, modifying workshops as appropriate and pre and Post-Assessment Feedback Surveys. Envision (Assistive Technology) is using Assistive Technology to bridge accessibility gaps and ensure full participation for people with disabilities in all areas of life. Stay up to date regarding assistive technology trends and products. Stay informed about assistive technology integration in mainstream society to enhance access for people with disabilities, provide educational workshops on assistive technology and work one-on-one with consumers to obtain assistive technology. Living Well: Empowerment through Healthy Living is a peer-directed whole person wellness program for people with disabilities. Participants develop an empowerment strategy by embracing healthier habits and strategies to enhance independence. Provides goal setting, problem solving, healthy reactions, beating the blues, healthy communication, seeking information, physical activity, eating well, advocacy and maintenance.

  • Eligibility Requirements:Coming Home/Thriving in the Community (Transition) eligibility is for individuals relocating from an institutional setting to community- based living. Youth in Action (Youth Transition) eligibility is for individuals with disabilities ages 14-24. Community Connections/Engage (Community Education) eligibility is for Individuals with disabilities who can benefit from educational topics and resources. Envision (Assistive Technology) eligibility is for individuals with a disability who could maintain or enhance independence through access to AT devices, equipment, or services. Living Well: Empowerment through Healthy Living eligibility is for individuals with disabilities who want to live a healthy life by creating and reaching exciting and meaningful life goals.. No Documents Required. Call for an appointment, no walk-ins. Potential consumers should contact the main number and will be transferred to the appropriate staff member.
